Instructions for completing the form


Line 1 indicates the total number of applications (requests) for the provision of state (municipal) services received from applicants - individuals directly to the body providing the state (municipal) service or subordinate organization, in accordance with the data of the applicants' appeal registration systems (for example, accounting books), including by appointment, on the last day of the reporting period. When calculating the indicator on line 1, statements (requests) received through subordinate organizations are also taken into account.
Hereinafter, subordinate organizations are understood to mean institutions and other organizations authorized in accordance with the legislation of the Russian Federation to provide the corresponding state (municipal) service.

Line 2 indicates the total number of applications (requests) for the provision of state (municipal) services received from applicants - individuals through the MFC, in accordance with the data of the applicants' registration systems (accounting journals, etc.), including by appointment, to last day of the reporting period. When calculating the indicator for line 2, statements (requests) received through the attracted organizations, provided for in paragraph 1.1 of Article 16 of the Law N 210-ФЗ (hereinafter referred to as the attracted organizations) are also taken into account.

Line 3 indicates the total number of applications (requests) for the provision of state (municipal) services received from applicants - individuals in electronic form through the Unified portal of state and municipal services (functions) in accordance with the requirements of the Regulation on the federal state information system "Unified Portal" state and municipal services (functions) ", approved by the Decree of the Government of the Russian Federation of October 24, 2011 N 861, on the last day of the reporting period.

Line 4 indicates the total number of applications (requests) for the provision of state (municipal) services received from applicants - individuals in electronic form through the Regional portal of state and municipal services (functions) in accordance with the Requirements for regional portals of state and municipal services (functions ), approved by Decree of the Government of the Russian Federation of October 24, 2011 N 861, on the last day of the reporting period.

Line 5 indicates the total number of applications (requests) for the provision of state (municipal) services received from applicants - individuals in electronic form through the official website of the body providing the state (municipal) service on the last day of the reporting period (in accordance with the credentials systems of relevant sites).

Line 6 indicates the total number of applications (requests) for the provision of state (municipal) services received from applicants - individuals in any other way of the information and telecommunication network "Internet", in addition to those indicated in lines 3-5.

Line 7 shall indicate the total number of applications (requests) for the provision of state (municipal) services submitted from applicants - individuals in any other way, including by mail, in addition to those indicated in lines 1-6.

Line 8 indicates the total number of applications (requests) for the provision of state (municipal) services received from applicants - legal entities and (or) individual entrepreneurs directly to the body providing the state (municipal) service or subordinate organization, in accordance with the systems accounting applications of applicants (accounting books, etc.), including by appointment, on the last day of the reporting period. When calculating the indicator on line 8, statements (requests) received through subordinate organizations are also taken into account.

Line 9 indicates the total number of applications (requests) for the provision of state (municipal) services received from applicants - legal entities and (or) individual entrepreneurs through the MFC, in accordance with the data of the applicants ’accounting systems (for example, accounting journals), including the number by appointment, on the last day of the reporting period. When calculating the indicator on line 9, statements (requests) received through the involved organizations are also taken into account.

Line 10 indicates the total number of applications (requests) for the provision of state (municipal) services received from applicants - legal entities and (or) individual entrepreneurs in electronic form through the Unified portal of state and municipal services (functions) in accordance with the requirements of the Regulation on Federal state information system "Unified portal of state and municipal services (functions)", approved by the Decree of the Government of the Russian Federation of October 24, 2011 N 861, on Lednov day of the reporting period.

Line 11 indicates the total number of applications (requests) for the provision of state (municipal) services received from applicants - legal entities and (or) individual entrepreneurs in electronic form through the Regional portal of state and municipal services (functions) in accordance with the requirements for regional portals state and municipal services (functions) approved by the Decree of the Government of the Russian Federation of October 24, 2011 N 861, on the last day of the reporting period.

Line 12 indicates the total number of applications (requests) for the provision of state (municipal) services received from applicants - legal entities and (or) individual entrepreneurs in electronic form through the official website of the authority providing the state (municipal) service on the last day of the reporting period (in accordance with the credentials of the systems of the respective sites).

Line 13 shall indicate the total number of applications (requests) for the provision of state (municipal) services received from applicants - legal entities and (or) individual entrepreneurs in any other way of the Internet telecommunications network, in addition to those indicated in lines 10-12.

Line 14 shall indicate the total number of applications (requests) for the provision of state (municipal) services submitted by applicants - legal entities and (or) individual entrepreneurs in a different way, including by mail, in addition to those indicated in lines 8-13.

Line 15 shall indicate the total number of positive decisions (issued documents, committed actions) taken as a result of the provision of state (municipal) services, in relation to applicants - individuals, on the last day of the reporting period. At the same time, the number of relevant decisions is taken into account directly upon the issuance of the corresponding result to the applicant by the authority providing the state (municipal) service, or by a subordinate organization, the MFC, or upon the fact of sending the result in electronic form, by mail, or in any other way.
If the body providing the state (municipal) service, or a subordinate organization, the MFC passes the appropriate action on the issuance (direction) of the result to another body or subordinate organization, the MFC, the number of relevant decisions is reflected only once.

Line 16 shall indicate the total number of positive decisions (issued documents, committed actions) taken as a result of the provision of state (municipal) services in relation to applicants - legal entities and (or) individual entrepreneurs, on the last day of the reporting period.
At the same time, the number of relevant decisions is taken into account directly upon the issuance of the corresponding result to the applicant by the authority providing the state (municipal) service, or by a subordinate organization, the MFC, or upon the fact of sending the result in electronic form, by mail, or in any other way. If the body providing the state (municipal) service, or a subordinate organization, the MFC passes the appropriate action on the issuance (direction) of the result to another body or subordinate organization, the MFC, the number of relevant decisions is reflected only once.

Line 17 shall indicate the total number of decisions taken as a result of consideration of the provision of state (municipal) services to suspend the provision of state (municipal) services with respect to applicants - individuals, on the last day of the reporting period.

Line 18 shall indicate the total number of decisions made as a result of consideration of applications for the provision of state (municipal) services to suspend the provision of state (municipal) services in relation to applicants - legal entities, on the last day of the reporting period.

Line 19 shall indicate the total number of refusals (negative decisions) in the provision of state (municipal) services, taken as a result of consideration of an application for the provision of state (municipal) services, in respect of applicants - individuals, on the last day of the reporting period.

Line 20 indicates the total number of refusals (negative decisions) in the provision of state (municipal) services, taken as a result of consideration of the application for the provision of state (municipal) services, in respect of applicants - legal entities and (or) individual entrepreneurs, on the last day of the reporting period .

If the body providing the state (municipal) service, or a subordinate organization, the MFC passes the appropriate action on the issuance (direction) of the result to another body or subordinate organization, the MFC, the number of relevant decisions is reflected only once.

Recommendations for work in GAS "Management"

Responsibility

1. In accordance with the Orders "On the appointment of those responsible for the provision of information", the placement of information in GASU is carried out by employees of units vested with authority from the city administration and having an up-to-date digital signature made by the UFK department in the Chelyabinsk region.


2. Specialists of the units independently monitor the expiration date of the EDS, in the event of the termination of the EDS, they also re-issue them in the Department No. 13 of the UFK in the Chelyabinsk Region.

3. Information on connecting new specialists to GASU, as well as re-issuing EDS is available on the city administration website: “Informatization” - “Development of information technologies and telecommunications” - “GAS Management”.

1-MU reporting deadlines

1. Quarterly reports shall be submitted no later than the 25th day after the reporting period:

I quarter (January, February, March) - until April 25;
II quarter (April, May, June) - until July 25;
III quarter (July, August, September) - until October 25;
IV quarter (October, November, December) - until January 25.

2. Annual report - no later than January 25, the year following the reporting one.

Report filling procedure

1. The methodology for filling out reports of 1-MU is set out in the order of the Federal State Statistics Service dated 01.13.2016 No. 5.

2. When filling out the quarterly report, “Section 2” (lines 12-39) and “Section 3” (lines 40-45) are filled in.

3. When filling out the annual report, Sections 1 to 4 are filled in (lines 1-56).

4. When completing “Section 1”, the following provisions should be followed:

• Line 1 of the report “Total staffing” - takes into account the staffing of the city administration + the staffing of UIA “MFC” (if UIA “MFC” is involved in the provision of the service) as of December 31 of the reporting year (as of December 31, 2018 the staffing city administrations - 1035 people, UIA "MFC" - 141 people).

• Line 2 of the report “Number of employees (employees) involved in the provision of municipal services”, - the number of employees of the city administration unit taking part in the provision of this service + the number of employees of UIA “MFC” (reception department - 90 people, control department - 11 people), in case UIA “MFC” is involved in the provision of services.

• Line 3 of the report “Number of employees directly interacting with the applicant” - indicates the number of employees involved in receiving and issuing documents.

• Line 4 of the report “The number of employees performing other activities related to the provision of municipal services, including the decision to issue a result to the applicant and not directly interacting with the applicant” - indicates the number of employees of the city administration unit involved in the provision of the service, not interacting with the applicant.

• Line 5 of the report “The total number of functioning places (windows) for the provision of municipal services, equipped in accordance with the standards for the provision of municipal services, in the body providing the municipal service, or in a subordinate organization” - indicates the number of “windows” in MIA “MFC” + 1 ODP window, in case MIA “MFC” is involved in the provision of services (as of December 31, 2018, there are 109 units in the MFC).

• Lines 6-11 are filled with “0”, because the services are provided free of charge.

Development of GAS "Management"

1. Information on the state and municipal services provided by the city administration is transferred to the GAS "Management" system from the Federal Register of State and Municipal Services (FRGU). In accordance with this provision, the updating of services in the GAS "Management" is entirely in the competence of specialists of the city administration units.

2. Specialists of the city administration’s subdivisions independently follow the news of the open part of the GAS “Portal” portal regarding the methodological focus or the deadlines for reporting.

3. In the event of changes in the operating mechanisms of the GAS Management portal, information technology and telecommunications management specialists are working on setting up workplaces for unit specialists within the framework of applications.
